ANNOUNCEMENT: Effective as of April 20, 2022: We are PAUSING new client requests, and will be re-evaluating in June, based on funding, as to when we will reopen the new client request form.
If you are a new (first-time) client, please review our eligibility requirements below and email the requested documents to us for verification. If alternative means of eligibility need to be discussed, please email us.

- PERSONAL IDENTIFICATION -- Please provide one of the following: valid driver's license, government-issued identification card, passport, visa or work badge
- CHILD IDENTIFICATION — Please provide on of the following: birth certificate or hospital paperwork, passport, baptismal record, vaccination recored or school record
- RESIDENCY IDENTIFICATION — If your personal identification does not show your current residency, please provide a piece of mail addressed to you, at your place of residence. If this is not possible, please email us to discuss alternative form of verification.
